To renew a retirement visa in BKK, is an appointment still needed at CW? I was hoping to go tomorrow, but their booking system shows end of next week as first slot. Are most people just walking in? (Sorry, this question gets asked a lot, but I couldn't find any recent replies).

Hello, Wed I will extend my Non-O Retirement visa. The forms they gave me at CW included the attached form which I've never seen before. Who fills out this statement? My landlord lives out of town. Can anyone attest that I DO LIVE in my own condo? Someone suggested you can put Self and fill it in yourself. Is this correct? Also, to be clear, now they only need: TMP7, photo, hand drawn map, bank statement and attached form. (At BKK CW) Correct? Thanks! in advance.
