You have to apply via the passport offices.

There is no 'form' per se.

Basically, you turn up with your ID and they take your photo - and that is that.

For a child, you'll need from memory their birth certificate and a copy of the house registration they are on. Both parents need to turn up to sign permission on behalf of the child, unless you have sole custody issued by the court (in which case you bring that documentation too). A foreign parent would be advised to bring along their passport.

Three days (and 1000 baht) you have your passport. 30 baht extra for express postage to your home.

For a minor, if only one parent shows up the other parent must have signed a consent form. I believe that is done at the amphur, so the form is legalised.
