It would be cancelled. He would need a re-entry permit to leave the country during the under consideration period that would expire on the report back date. So he wouldn't be able to use the re-entry permit and would be stamped in as a Tourist under the visa exempt scheme (assuming he qualifies).
 
If it happened, and he arrived back only a few days after the report back date, it might be worth going to the immigration office to see if they could do anything, but they almost certainly won't play ball. He would then need to apply for a new non 'O' and apply for the extension again.

What happens if one is out of the country during the under consideration period and he can't return on time to get the year extension final stamp, is the application canceled or they put it on hold until he's coming back?

The last time I've been there they did not ask me for this document... neither to a friend of mine that went there last week... more confusion under the stars...

Did in past and just did it few days ago and no letter of authorisation is required in Phuket... 

Does anybody know how many and which Immigration offices are strict like phuket on this matter? Friends elsewhere have told me that they are not required to report so frequently, they did it just a couple of years ago when they enforced this law...

I believe this is just for hotel's owners purposes, they can notify the presence of their guests through this.
