Phone charger is 4, 9, 12 volts output. His extention( 220.v ) may have been faulty. 

Find yourself a reliable  immigration / visas,  office , from recommendation if possible. They will guide you and do all the  leg work for you. All you will need is to show up for your picture. They do have reasonable fees and will save you many headaches. They also know the proper " channels "... Thai immigration nowadays has become a real pain , and is only getting worse. Good luck. I would personaly stay away from BKK.. 

I was told once that those remote controls  for barking dogs can work on some sound system  equipments .not convinced but who knows..
