Also you said in OP it only gets you 90 days on entry. The new elite visa gets you one year on entry. Much more useful and at 900k baht for Initial 5 year visa much cheaper (1.5 million for 10 year or 2.5 million for 15 years)
It's about the benefits and the ability to transfer. Everyone would have to make their own decision on what they would prefer. Prices will only keep going up, so in theory you could use it for x years and resell.

Lifetime is only to describe the visa as only the original lifetime visas had these perks.

But with 15 years left you can convert it and only have to extend annually. In fact you can do it right away but you would lose the 5.5 years

There is 20.5 years left. The original Elite visa's were lifetime, and transferrable. I bought it from someone who in turn bought it from the original holder. He used 5 years and sold it to me with 25 years left on it.
If you look at the current elite offerings, the 15 year is 2.5 million, the 20 year is by invitation only and is 5 million baht. Since this visa is not based on the current point scheme, you can go for a massage or play golf everyday. As an example, a local high end spa charges 2800 baht for their 1.5 hr Thai massage, and the visa holder can attend this free daily.
Elite also offers to convert this to a 15 year visa with the same perks, so once you hit the 15 year mark, you could choose to convert it, but in my opinion, you would get less perks especially if you really use the massage/golf/airport transfer a lot.
I priced it exactly like the current visa's. Pay 900K for 5 years, then 2.5 Million for another 15 years. My guess is that in 5 years, the prices of these visas will be even higher.

You can't choose the consulate, it's based on where you live. Depending on the state, you will get assigned to one of the four embassies.

What bank is it? I faced this problem a few times so I am becoming an expert.
FYI the $3 TMobile sim is now called a paygo ultra mobile sim, only sold in select Tmobile stores.. I had to hunt it down on my last trip, and now will just keep paying the $3 forever. They will NOT sell it online, but I've seen folks sell this for upwards of $50 on amazon.
Google voice is a good option as it's recognized by Bank of America, other VoIP won't be, it's all bank dependent.
Best option, find someone you know in the US, have them receive the SMS to get. you over the current hump, then have them buy/activate, then mail you the ultra mobile sim (specific one called paygo). I've tried many of the apps, but the banks know what falls into the voip bank. There was an online service to receive SMS's on an actual sim and you pay per sms, but I lost that link.

I just booked this same route, BKK->TPE->LAX. There is an earlier flight out of BKK that makes this a 5 hour layover. I was checking and China airlines is late a lot from bkk->TPE so you will miss that connection.
