Before diving in, there are a great many places where you are free to smoke (a good 50% of all venues outdoors or on rooftop) but the below are a list of places that either sell cigars to smoke on-premise or to-go (the latter marked as "SHOP ONLY")

• Aqua at The Four Seasons Bangkok : CLOSED (though see "Character Bar" below)

• LCDH/ La Casa Del Habano in the Bamboo Bar @ Mandarin Oriental Hotel :: Top 3 (No smoking in the proper bar but the lounge for smoking is just adjacent through glass windows and next to the band. Full bar service available from your seat in the LCDH lounge.
• Black Amber in Thong Lo : Excellent period style cocktail bar with live music, food and good priced quality cigars.
• Red Sky Bar @ Centara Grand Hotel rooftop
• CigarOne (Near Conrad Hotel) : Modern space run by mixologist with concise, curated cigar selection
• Cohiba Atmosphere (Also near Conrad Hotel) : Top 3 Perhaps the best selection in the city. 
• Duke @ Gaysorn : Top 3, quietest of the top 3, also offers some non-cubans and you can smoke your own if you bring the receipt from the Davidoff shop below
• Davidoff @ Gaysorn : SHOP ONLY, good NC selection as well as Cuban
• El Guacho @ Asok, near T21 : Smoker friendly restaurant/ lounge with limited but good cigars 
• Flamenco @ EmQuartier : Outdoor venue with limited cigar selection
• GodFather @Central Embassy : 
• Indigo @ Silom/ Sala Daeng : Fine dining restaurant with smoking parlour on 2nd floor.
• Character Bar @ Anantara (essentially replacing Aqua) : Great atmosphere with great selection of cuban and NC
• Character Bar @ Compass Hotel : Newcomer that is amazingly stocked and classy, on a high flor near the roof bar/ club. 
• Lobby Lounge @ Intercontinental Hotel 
• Hanuman Bar @ Siam Kempenski Hotel 
• Pacific Cigar Divan @ Crowne Plaza Hotel
• ThreeSixty @ Millenium Hilton  : Outdoor bar with cigars on the menu. EXCELLENT VIEW.
• Whisgars @ Multiple Locations : Limited, non-cuban selection only. I can't say I really like these places. The cigars are ones you'd often not even smoke in America but at 3 x the price. The service is often uninformed too. But I can say that the environment is the most casual and even Hard Rock fun-campy. Reminiscent more of an American blue collar man cave than a stuffy room of people wearing tweed in the tropics. Music is often lively and the drink selection is generally good. It's for casual smokers but/ and it's enjoyable.
