I was vaccinated at CVS in the USA. I received the standard COVID-19 Vaccination Record card from the CDC that Sheryl posted a picture of in this thread. I scanned it and submitted it for my COE and had no problems. My application was through the Royal Thai Embassy, Washington DC. You can always exchange emails with the embassy if you need to. They seemed to always be responsive during the process.

Yes, as DrJack54 stated, there was a long queue of taxi cabs at MTT. Also the taxi cab that delivered us seemed to know where to go and drove right up to the building.

Thank you everyone for your responses! I was able to get my extension at Muang Thong Thani Counter K (3rd floor) very easily today. I arrived at 12:15 PM and was out of there by 12:45 PM. The signs said it was closed for lunch at that time, 12 to 1 pm, but they were working anyway. I only saw one other couple who were getting paperwork done at the same time as me. I filled out a form they gave me (TM7?) and glued my photo to the back of it. I also gave them copies of my passport info page, my passport stamp and the departure card (front and back). I was able hand over my passport at the copy window on the first floor to make those copies. Cost maybe 9 THB? One person checked my documents, then sent me in to get processed once I got all the paperwork right. They took my picture and I paid the 1900 THB. Five minutes later, they called my number and handed my passport back to me. Done.

I am an American in Thailand currently on a 45 day visa exempt stay and would like to extend my stay. I am seeing a lot of conflicting information. First, where do I need to go for this? I have seen references to Muong Thong Thani (Counter K) and also Chaeng Watthana. Which is the correct location? Also, I have seen one site say the extension is for another 45 days and some saying it is 30 days? Also, what papers do I need? I have seen that I need a passport photo and 1900 THB. Do I also need new COVID-19 insurance paperwork to cover either the 30 or 45 (which one?) days for the stay? My stamp ends in 9 days. Thank you for any information you can provide.

No, they did not. I was staying at a hotel in Bangkok and wrote the name and address of the hotel on the form I filled out. When they checked my paperwork, they asked me to add the room number at the hotel. No other mention of address after that.
