Thanks. I tried looking for this online, but I cannot find a registration certificate/form for medical devices.

Until Monday, I have sent my friends and family members certain non liquid vitamins/supplements sold in Thailand via Thailand Post international airmail, but yesterday, I went to send some like usual, and they said I could not send it without this paperwork; they do not know were I can get it, all they could do was give me an example of the paperwork. I cannot find this anywhere online. Does anyone know where I can get this from? Thanks.
