Thai Passport Office In Chiang Mai

I got my Thai daughter a passport at the government office in CM a year or so back. It went without a hitch, but they sent it back to her home address in Bangkok.

Now I want to get a passport for my Thai girlfriend. She lives in Petchabun. So two quick questions:

1) Its just the Thai ID card needed isn't it, not house registration papers as well? (My memory not what it was)

2) Does the pasport office always send it back to home address, or is there option to pick it up for the passport office. I would much prefer the latter as we plan a trip out of the country about a week after doing the passport application.

Can anyone confirm please whether an adult Thai needs house registration papers to make a new passport or just Thai ID card?
