I just got my second jab and told to get a passport you required vaccination certificate. Passport and flight tickets and 50 baht

After jab 2 you get a piece of paper with a QR code on it. You need that and your passport. I only know where to go in Chiang Mai. If you say you are not travelling yet no flight details needed. Collect one week later and pay 50 Baht.

If you scan the same QR code in to line you will also get a digital copy of your jabs but not sure what this can be used for.

Did you have to make an appointment? Or just walk up with vaccine certificate/details?

Walk in. There is a car park to rear. Two desks open, you need the office directly facing the road. Think we only took passport and the bit of paper you get after second jab.

Incidentally if you scan the QR code you get on the paper after the second jab in to Line you also get a snazzy version of your vaccination records

I copied and pasted the below from another post of 4 hours ago compliments @BBKBike09 which is pretty self explanatory.

The following video from the MOPH CDC explains in simple terms how to apply for the Travel Certificate. The video is in Thai but has English sub-titles. It makes it sound simple. While it is not hugely complicated, it is quite time-consuming because it requires in-person application, unless you can authorise someone to apply on your behalf. One of my staff did it for me, at the main MOPH building. I believe it cost THB 150 in official fees.

Plenty of my mates here in Chiang Mai have these and I have been with them when applying. Simple and took about 5 mins. You do have to do two visits. Apply day 1, collect in a weeks time 50 Baht. Has name, passport number and jabs received on it. Plenty of room for boosters.

What proof of vaccination can be used for those vaccinated abroad? I have a vaccination certificate from france as well as the EU vacation certificate. Are those of any ise in Th?

I went with a couple of mates who were collecting their 'vaccine passports' here in Chiang Mai. I asked if I could get one having been double jabbed in the UK. Got a polite No.

and what was the the validity  3 months from issue or date of first or second jab please? Or does it depend o the vaccine?

OK so picked up the vaccine passport. I can not get one as I was vaccinated overseas and I do not have permission to show an image of the one we got.

It has your name and passport number on the front page. Hand written.

It has 2021 03 followed by another number also on front page. Maybe a date and then the document number. This is a guess as it does not state exactly what it is.

Inside are the details of your jabs with plenty of room for more.

Specifically on validity it states in the notes ' The validity of the certificate shall extend until the date indicated for the particular vaccine'

Apologies if this is as clear as mud. I tried to confirm what the dates meant but got nowhere. Should have taken a Thai speaker with us.

Ive done LOS a dozen times with one way tix but only really been questioned once and still allowed me to go..never gone with a visa either

I prefer to have the ticket, even just for reassurance. Also I get my plans queried often (have sat in the naughty chair at immigration in Singapore and Bali for a 'chat') I even get my hand luggage weighed.

    Must have a guilty face!

Elaborate please...never heard of renting tix--only girls in thailand ????

Hopeless at providing a link but check out www.bestonwardticket.com

One of many providers. Process is also explained on home page.

I nearly always get one of these if flying one way. Only had show twice at check in.

  8. People will be on to recommend firms like Transferwise etc. I would add if comparing companies then look at the rate not just the fees. A low fee is worthless if the rate is lousy, just a fee by another name.

Also tell your UK bank before you transfer. I have the misfortune to have HSBC as my bank and regularly get relatively small transfers blocked due to 'possible fraud' A real ball ache to get fixed.

Did you ask whether we can skip the 14 days quarantine in Thailand with this Covid passport if we return back to Thailand after a short trip abroad?

I read that the vaccine passport is only valid for 3 months from issue date. Is this true?

Anyone arriving in Thailand today, vaccinated or not, gets to do 14 days quarantine either in a hotel or via the sandbox. You also need insurance and a Certificate of Entry. Having a vaccine passport will not change this. Can not comment on tomorrows rules just yet????

Will confirm validity when we pick up next week.

May I ask where in Chiangmai ? This would be useful information.  The notice I have read says to go to Bangkok in person.

I understand there are a few places you can go but we went to the health clinic opposite a Siriwong Book Centre on Sridonchai Road. Where the graphic asks for flight details just say you live here so not needed. Any other details needed just shout, will reply when I get back from beers.

Can you give me another landmark where this clinic is? Across from the book center?

To finish up. Yes, the clinic is directly opposite Suriwong Book Centre. There is a small car park at the front and a larger one behind on the left hand side. There may be other places to go, not sure.

You have to go to the office at the front facing the road. Not completely consistent with what you want but definitely copy of front page of passport. One mate had a copy of his Wall of CM registration with the 13 digit no, one did not. As with all things Thai take a copy of everything!

Very easy, 5 minutes max we were the only ones there.

Receipt provided with a date to collect in one weeks time.

I did ask if I could get this passport having had two jabs outside Thailand but got a polite 'no'

    Best of luck

OK. I managed to install on my phone since I can't install on my computer.

Luckily, I know some Thai because there doesn't seem to have much English on the app.

I saw my health certificate there but how do I download into my phone? I don't have wifi all the time.

Is this the same as the Green card required to enter restaurants in Bangkok?

Sorry but I am not the technical bloke to ask. If you can see your vaccine records then maybe take a screenshot on your phone if you are worried about no wifi.

I am going to the health clinic on Sridonchai tomorrow with a couple of mates to apply for the 'vaccine passport' hardcopy so will report back if I learn anything new.

I guess either of these will work if the rules for dining in in deep red zones is actually enforced.

If I seem a little vague on details I am just trying to help some mates out, both my jabs were done outside of Thailand so does not really affect me directly.

Could you give me the link to the Mor Prom app? Can we download it onto our windows computer instead of cellphone?

The Mor Prom app is available in the Google Play Store. As long as you have that see no reason why you cant download to a laptop etc. Being a computer dummy not sure how to provide a link but if you just type in Mor Prom in to the store it is the top one. Green background with a white cross. 5m odd downloads. It does say Mor Prom but in Thai. Good luck.

Never used any mor prom app, is there an English version? Which QR code from where?

I am just wondering whether the old fashioned vaccine passport has an expiry date for travel?

I plan to travel end of year or early next year so if I apply now, would it be made invalid?

The Mor Prom app has English instructions. I helped a mate register today. Two computer dummies and we managed in 5 mins. All his details there, he only got his second jab yesterday.

Needed his Wall of CM number starting with a 6 and a phone to receive a OTP.

The QR code is the one on the certificate handed out after the second jab.

Had lunch with a mate who had the vaccine passport. Think he said it was valid for 10 years but don't quote me on that.

Huh? I didn't get any sore shoulder. I felt a slight headache for a few hours after that but I am feeling fine now.

I already got my certificate of vaccine yesterday although they said the online system was unstable.

Just for info. After your second jab you can use the certificate to get your details on line by using the mor prom app. Scanning the QR code in to Line also provides an on line record.

If you want an old fashioned 'vaccine passport' take your certificate and passport to the health clinic opposite the UOB bank on Sridonchai Road. 50 Baht, takes a week.
