  1. My surgery was Seattle, WA as my "US residence" is Sequim, WA. The surgeon DR Kathleen Stickney received her parathyroid surgical training in Tampa at USF (Norman method).

     

    Felt better right after surgery, calcium returned to pre condition level of 9.0 only problem was osteoporosis from the 5 years of it not being diagnosed. I had labs/physicals done twice every year once here in Thailand and once in the US and the doctors always just said "your calcium is a little high'. 

  2. I had the surgery 5 years ago. At that time there were no surgeons in Thailand doing the minimally invasive procedure so I had it done in the USA . The surgery was outpatient and took less than 20 min. Before going to the US I saw two endocrinologists in Thailand one at Samitivej and one at Bangkok Hospital. The doctor at BH wanted to also have a Sestamibi scan done for confirmation. I told him that the surgeon would do the scan as she used it for location not diagnosis.

     

    The diagnosis for me should have been easy - high yearly calcium 10.5 with high PHT.

     

     

     

  3. As you plan to go analog out from your Denon the speakers for the 5 channels should be 5 equal full range speakers (18K Hz to 40 Hz). The reason is that the MC SACD's are mastered to produce full range sound from each of the 5 channels.

    An alternative would be to go HDMI from the Denon (if it supports SACD) to an AVR that has equal or better DAC's then to the HT speaker configuration you listed using the AVR's Audyssey MultiEQ Auto Setup and Room EQ.

  4. I listened to all you "experts" that can read and quote the regulations that a medical certification is no longer required when I went for my annual visa extended two years ago at Siracha. When they said I need a medical certificate I told them that it was no longer required. They said they are aware of what the regulation requires and that they have the right to request additional information and they want a medical certificate. Now, whether they have the right to request additional information or not under Thai law,I don't know and I don't think any of you do either. So I think the prudent thing to do is just ask the immigration office what is required in advance.
