I did mine two weeks ago at VFS in Chiang Mai. Just book an appointment online and photocopy everything yourself. It was very easy and stress free. The staff were very professional and helpful. My passport is already done and I'll be picking it up this week.

I've been working on the premise that photos with a watermark on the back are unacceptable which was the case the last time I got a new passport. Someone told me that this is no longer the case and paper with a watermark is acceptable now. I will find out on Friday if this is correct when I go to VFS.
