That is the latest version of the form.

I never knew of anyone that lied and I certainly never did but I still have to believe on a percentage basis that the numbers are low- The Embassy could have eliminated many by simply asking for documentation and then Thai Imm - if suspecting an issue also ask for the proof.
I was witness to an undignified performance by an American who was asked by an IO to evidence what was stated in an affidavit.

You can apply for an extension up to 2weeks early. The extension dates from the end of your current permission to stay(stamped in your passport). The visa is used and is of no further use.

If you do not fit the profile you will not be asked.

No one is "forced" into making application for an extension of stay but the cost of an extension(1900Bht/year) is cheaper than buying long haul flights + the costs of an O-A visa.

Yes - you can apply for an O visa based on retirement at an immigration office if you meet the requirements for a subsequent extension of stay.

if you move province then submit a change of address/TM30 to the relevant IO who will then process your request for an Extension of Stay.

The OP said, "I am due to renew my retirement visa in 10 days"(sic)- What are you having a problem understanding? No mention was made of an extension or 'renew "permission of stay based on retirement", hence my question.
You might be an "expert" clairvoyant but most are not.
