I have never forked out a dime with mine or my families health insurance at any hospital we have used in Thailand over last 20 years or so. We present our insurance card, they contact the agent in Thailand and that's it I think the largest claim was circa 700,000.

So if my wife and I book to tickets to Thailand through say 'skyscanner' her ticket will be 300 baht cheaper than mine? I have a feeling Thais as well will be paying this extra as well. There will be no difference in price.

Yes and No. When dealing with government and officialdom is generally a No. They always seem to be quick 'difficult' (but then again this is also the same for a lot of Thais as well). With people in general yes. I have found that if you can speak the lingo, or at least make an effort to integrate most people will meet you half way.
