Yup.. Broken, at first I thought it was because of te chrome update, but no.. 

It does not matter if you mail it, do it online or report in person,  they will start the new 90 days from the date you do the report.

Some offices do not do (accept) online reports, yours seem to be one of them judging by the officers comments.

Yes, I have the same problem, I didn't even know there was still supposed to be a text there, I just thought the whole `box` malfunctioned.

Got the same, also on Chrome latest version and windows 10 updated to the max.

125% screen zoom it is a mix, 110% zoom and lower it looks fine.

What office are you reporting to that wants everything you wrote about.

You are of course right UJ, it is just that some officers .... On the safe side it is convenient to have those copies just in case. I don't do 90 days reporting myself but have noticed the various differences, also discussed on this board. Playing it safe never hurts.

Permit to Stay Up until (Last Visa Stamp or Extension) TM.6 (Arrival/Departure Card)

Last receipt of Notification (Original)  Sign all copies.
Completed form TM 47 

Some offices also want to see some proof of address, like a rental contract or a utility bill. Most common is just show the originals. No money. And like with everything in Thailand it all depends on the immigration office. Some are happy with a completed TM47 only and a show of valid stamps in passport and the old (previous) 90 day report. 

You said there are two units with the same specs, have you tried switching remotes? It is a long shot but remotes sometimes can send erratic signals. Make sure the remote batteries are ok.

Suing for such a low amount is the ultimate insult.

After enlarging the pic on screen and noticing the shape of the beak I would say it is a raptor, but the pic is unclear to determine which one.

When reporting in person you have window of 15 days before until 7 days after the original report date.

Include the report date as the first day for the 7 days after calculation.

With 3 days late you will be fine. No problems there.
