I am a Pakistan national, and hold Pakistani Passport. For travelling to Thailand we require to get visa from Thai Consulate in Pakistan. As matter of routine, I got a normal single-entry tourist Visa from Thai Embassy in Pakistan.

At the moment I am in Thailand since last 10 days, and I still have time available for me.

Now I wish to make a short tour to Malaysia for 4 -5 days, and then come back to Thailand for 5 to 6 days before leaving back for my home in Pakistan.

I will make short trip to Malaysia ONLY if I get a reentry permit to Thailand, otherwise I will just pass my time here and go back.

Please advise me that can I get a re-entry permit of Thailand ? If yes, then whats the procedure , requirements and cost ?

Go to an immigration office wherever you are in Thailand and get a Re-entry permit, cost B1000 and some passport photos. When you return to Thailand from Malaysia you will be given permission to stay until the date of the original stamp (using the remaining days of your tourist visa)
