June 27, 20188 yr I'm looking for a hospital in Bangkok where I can get a real prescription letter for meds with my name, birth date, diagnosis and medication in case I am hassled at Customs in some country. All hospitals I've been to just give the standard "medical certificate" with a diagnosis (sometimes non-specific like "he came to me with facial pain") and recommendation to stay off work for X days. The medicine packaging has my name and hospital number, with the name of the med but often not even what it is for, let alone a proper diagnosis matching the medical certificate. At present I need this for a normal prescription med, not a psychotropic or controlled drug. But it is an "off-label" use for the condition I have.
June 27, 20188 yr I suppose if the ask the medical practitioner for a proper summary of your conditions with the relevant diagnosis and meds prescribed they will obliged, all you have to do is tell them why you need it and pay extra for it if they ask...
June 27, 20188 yr Thailand does not have prescription pads etc. You need to ask the prescribing doctor for a letter along the lines of: "To whom it may concern: Mr. XX has been diagnosed with (condition) and has been prescribed the following medication for it: Drug name dose route oral etc) frequency Dr signature and name" This is usually not hard to obtain and there is usually no charge for it but you must discuss it with the doctor, if you just ask the clerks or nurses they will give you the standard medical certificate because that is what they are used to. It will help if you provide them a written sample with the necessary wording.
