Which Form is the one that has to be signed warning extension can be canceled

Every time I apply for a retirement extension, at Jomtien, they give me 2 pages that warn me the extension/visa can be canceled if I break the law or do something bad. The forms need to be signed and submitted with the application package. Where can the forms be downloaded ? All form titles at the immigration website are in Thai https://www.immigration.go.th/download/

It is very difficult for a foreigner to make a complaint about a Thai person since the foreigner is always wrong and this could lead to the cancellation of a visa.

I had to sign that form in May this year. 

I vaguely recall having been required to sign those forms several years ago, but didn't have to this year, and think I didn't have to last year. If they need them they'll have blanks available for you.

I had to sign them last week when I submitted my paperwork.

Everyone has to sign it. The IO prints it off and passes it over for your signature. Maybe you're simply one of the many who just sign and pass it back without reading it.

 

I read through it the first time 5 years ago, but I've never bothered since.
