Is a visa required for the Non-o initial 90 day visa?  I will transfer this to a 1 year marriage visa once arrived,  I know the 1 year visa needs insurance but not sure about the 90 day one.

Only a non-oa long stay visas issued at embassies and consulates requires insurance.

No need for insurance to apply for a non-o visa or to apply for a one year extension of stay based upon marriage.

Maybe he meant did he need to enter on some kind of visa in order to get a conversion to a non O, but I agree with you, calling everything a visa (including the extension based on marriage) and claiming "I know" that insurance is required implies he hasn't made much effort to understand the process or he's getting information from some confused source.
