If you are going to Laos and coming back to Thailand the same day what do you put on the hotels/guesthouses part of the form.

Do a search and find a hotel name and address to put on the form. They don't check or care.

When i did this in Nong Khai, i simply put "Vientiane" in the address field, was always accepted.

If your coming back to Thailand same day you leave that part blank and just tell the guy in the window who is processing your Laos visa that your not staying in Laos and coming right back to Thailand as that's what I do every in and out 90 day Lao boarder run for years.

i usually wrote the same as when entering cambodia or thailand......"guesthouse"
