First had the fisure problem in the 80's had a surgical procedure and no more problems until three years ago. Now it's on and off, heals up for a while then tears open again. I know all the advice about water intake, fibre and exercise etc. Been using the nitrate ointment from the Dr back home, I don't think it's available here. Friend in NZ has had botox injections in her bum to relax the muscle and help healing, I was wondering if anyone has heard of this being available in Bangkok. She said after years of suffering with it that it worked wonders. Dont fancy it but even that would be preferable to the pain it can cause.

Sounds like a real pain in the arse. You need a 

lateral internal sphincterotomy.
