For a Thai person you just take your passport to the passport issuing office. The original page, name and photo is left the same. But they complete an "amendment and endorsement" page in the passport (usualy on the next page). They show the change and officially stamp it. Quite easy to do.

Any time in future you use it, you may have to point out people that the original name has changed, and show them the next page together with the first page. People often don't expect to look for an amendment, eg airlines, immigration.

There is a passpot issuing office in Bangkok in the Central, Bang Na complex. Think it's 4th floor, but you'd need to check.
