YuriGuest2010 Posted August 31, 2010 Share Posted August 31, 2010 you need to photocopy your passport first page with the picture on it and copy of the immigration stamp on your arrival .Photocopy the TM arrival card too and phtoto copy the page with your last tourist visa too. you need to fill up the application form from the Immigration and paste a photo with it. However I did read on another forum that the photograph should never be glued or pasted, just handed over loose so the Immigration Officer can staple it to the A/F.
