Recently got a new multi entry o visa but now my passport has only 1 page left and will only last 1 more visa run before its full. Can I get the visa transferred into a new passport? Can I do this in Chiang Mai?

You cannot transfer a Visa to a new passport. Only Immigration stamps can be transfered.

Carry both passports when travelling.

If you get a new passport make sure they don't damage the pages with the valid visa on it. That one remains valid in your old passport and you will need both passports while the visa is still valid. The old one to show your visa and the new one to get your permission to stay (the stamp at the airport). Once the visa has expired you don't need the old passport anymore.

But once you have the new passport you need to have your entry details to be transferred to your new passport. It is free.

Ok sounds good, I thought you had to surrender your old passport when applying for a new one but just double checked on the website and they say a copy is ok;

Specific requirements from Thailand

ORIGINAL PASSPORT: Thailand is an exception to the main guidance given on our primary website. It is not advised to be without your original passport in Thailand and we recognise this. You may therefore submit a clear photocopy of your passport (just the page with your personal details and photograph). Please note that the passport being renewed will be electronically cancelled and is therefore not valid for use as a travel document from the moment you submit your application. On receipt of your new passport, you must physically cancel your old passport by cutting the corners of the photo page and the front and back covers. Failure to do so may leave your replaced passport open to abuse. Alternatively, you should visit the British Embassy in Bangkok to have your replaced passport physically cancelled.

I assume immigration in Chiang Mai can transfer my permission to stay stamp into the new passport once it arrives.

On this when I have a new passport then do I need to visit an immigration office prior to crossing out of Thailand or will immigration at the border transfer it for me provided I have my old passport?

People seem to be able to do at border having both passports but will take a little extra time. I would, and do, visit immigration as directed by Embassy and have it done there. Not doing much travel makes it the logical thing to do.
