I need to do the 30-day extension and would like to clarify some questions on the TM7 form.

On first page:

1) Arrived by (mode of transportation) _________from

2) Port of arrival ______ date____, month_____, year_____

Question: what should i fill in here? what are they referring to?

I've got this Double Entry Visa in Loas via Bus, but before this, i arrived BKK on an exempt visa via Air.

On second page

1) Address in Thailand__________

2) This application is writtten by __________ address no__________ road______Tambon/Khwaeng_______Amphoe/khet________Changwat____

3) Signature _______ Write

Question: is the address of 1) be the same as 2) ?

Question: what are the last 3 fields of 2)

Question: am i supposed to sign in 3)? becasue there's already a space for applicant's signature on the first page.

My Immigration Stamp at Mukdahan indicated 12 Oct as admitted until 10 Dec 2011,

a) Does it mean my input should be:

Arrived by (mode of transportation) bus from Laos

Port of arrival Mukdahan date 12 month Oct year 2011

On page 2 (address)

what is Tambon/Khwaeng , Amphoe/Khet, and Changwat ?

A is correct.Tambon is the place (town/village/sub-district) you stay in, amphur the name of the district and changwat is the province you are staying in.

Ok. Got it.

Since 10 Dec 2011 is my last day, what is the earliest date that i can submit this application?

You could try 30 days early, but normally for an extension on a toruist visa entry one applies 1 or 2 weeks in advance.

If I applied 2 weeks in advance, will it affect my next border-run, ie i'd have to do border-run earlier than required?

You donot lose any days when you apply early your extension will be from the expiration of the 60 days.
