Hello All,

I have tried a number of different methods but cannot seem to get hold of a copy of the captioned application form in a correctly formatted file; it just seems to be all over the place.

Can I ask if any member has a copy where the formatting is at least intelligable enough to be able to fill out neatly please? Or is that just the way it is?

If using the Doc file you need to use Microsoft Word or the free Kingsoft WPS to get correct format - the popular alternatives such as open office and such do not work well at maintaining formats.

Thanks. I am on Mac so saving the file to Google Docs and editing through that...

Thanks I have tried that but they do not open in the correct formatting. The files supplied by jpinx are however, correct. wink.png

Make sure you print it as a two-sided document (front and back), not on two pages.
