Cancelling My Extension

Does anyone know if there is a form I need to fill when cancelling the extension of my Non-Imm O Visa? I have a letter from my employer (a foundation).

I need to cancel the Visa before leaving to apply for a business visa (have all the peperwork for that). I just can't seem to find a form on the Immigration website.

A link to the right download would be wonderful.

There is no form from immigration.Just bring the letter, clearly stating your last day of work (that is the day you have to elave the country or are on overstay if not).

Do not forget to also cancel the work permit.

when you leave the country the visa is cancelled if you do not have a re-entry permit so I am sure that is no problem

Thanks guys. I went to Immigration this AM. They wanted the letter from the employer, copy of his baht prachachon and a photocopy of the relevant passport pages. It took all of five minutes. Quick and easy.

By the way, I had my employer request two extra days for me and they stamped it right in without asking for 1900 baht.
