Hello,

I'm aware this topic has been beaten to death, but couldn't find what I'm looking for.

We all know what we have to submit to renew the extension of stay stamp under marriage. However, every time I submit my application, it seems two documents are always missing. One of them is a sort of acknowledgment act we have to fill in and sign. There's also another doc I guess but I forgot its purpose.

Do you know if I can download (or pick up at CW Immigration) the acknowledgement paper so that I can filled it in before hand and so that I don't waste any unnecessary time?
