What I have heard you just go to the place you got it originally. You should be in they system, would be better if you have a copy of the license.

From other reports:

The police report plus passport is about all you need.

A copy of the lost DL can not hurt.

No full paperwork needed.

Best go to issuing DLT office.

In and out in minutes for some.

Replacement will of course have same expiry date as the lost one.

Lost mine a few years ago, police report went out to the office on the 36, new license issued no problem.
