Looks like the problem was with the photo... I combined visa and passport photo... and when submit only passport all works fine... May be they have validation of the image quolity, size and etc...

There are other symptoms which i described in the initial post. Also i see that internet connection is marked with "no internet access" when breaking duration is significant.
