One completed TM7 form with a 4 X 6 cm photo attached. Download, complete and print 2 sided this Fillable TM7 form.pdf .

Letter from bank confirming your account and balance done no earlier than the day before you apply (they prefer same day). Copies of bank book updated on the date your apply.

Copies of passport photo page, visa, entry stamp, current extension (if any) and TM6 departure card.

Proof of address if first application there.

Hi Joe, additional question on renewal of extension -- how early can I do it?  And will it then be valid a year from the date of the new extension being issued or a year from the original visa expiration?

At Chaeng Wattana and some other offices you can do it up to 45 days early. At other offices it is 30 days. The new extension will start from the date you current one ends. You lose no days by doing it early.
