How Can A Costa Rican Get A Tourist Visa For Thailand?

I imagine they have to get the appropriate form, the required documents and post them to the appropriate Thai embassy. If that is the case my questions are
 

* What is the appropriate form?
* What are the required documents?
* Which is the appropriate Thai embassy?
* Would it be 60 days with a possible extension to 90?
* Is multiple entry an option?

You can apply at the honorary consulate in San Jose. Form is on their website.
