Early morning flight tomorrow 0655 ex-Chiang Mai connecting all the way through via Bangkok to the UK.   Do I get the re-entry permit at Chaing Mai or Bangkok?  

(If bangkok, where is the immigration office?) 

I will need the boarding pass, the TM8 form, cash 1,000 baht, passport, passport pages copied, photo - right?

Well I would have gotten mine before hand at Immigration office,

but I'm a belt and braces man,so dont leave things to chance,too

many things can go wrong, 

I would try at CNX first,if cannot get there,you will have to enquire

where to do it at Swampy,   Good Luck.

You will see the re-entry desk just before security check at CM International Departures .  If it is a flight booked from CM to the UK you will be in the transit area in BKK and the re-entry can only be done in CM.
