I usually go to Bumrungrad to do my 90 day report (quite good).

 

This time, my girlfriend will be the one going since she have to do some other business at OSOS Boi Chamchuri sq too.

 

Is there any special authorization form I need to sign before she can go for me?

Just a short note stating she can do it for you incase it was asked for.

My wife does mine. I just print out a completed TM47 form sign it and give it to her with my passport. It has never be a problem for her to do them.

My local office (Samutprakarn) will not allow someone else do a 90 day report unless they have a signed Power of Attorney form.

I am not clear whether the Immigration office issue the POA form.
