I'm filling out my application now and need advice of what to put. I'm doing a 90 day trip so ill be extending my visa for 30 days. Where it says "length of stay" would I be putting 60 days, or 90 days? And where it has contact information it asks for home number, cell number, office number.. I only have a cell phone. Would I put N/A next to office and home number, or would I just leave it blank? Not sure because at the bottom it says the application needs to be fully filled out

Length of stay is 60 days.

 

Just leave blank if you don’t have home/office numbers. They wouldn’t be checked and aren’t important.

thanks and i booked a hotel for the first 2 nights, when it asks for my place to stay, I just list the name of the hotel and address of it aswell as their phone number, do they even check this stuff? Like do they call the hotel and check how long I'm booked for?

No they don’t check anything. 

 

It’s a generic form and many of the questions are irrelevant for a tourist visa application.

I'm about to go to USPS now. Could you tell me what I put for the references on the application? It wants a reference person and address in USA, and a reference person and address in thailand

You don't need to provide any referees. Just leave blank or write n/a.

thanks and last question: for the signature / date area at the bottom of the app, do i sign it on my PDF app or do I print it out then sign it and date it with a pen?

Better, IMHO, to use a wet signature.

I would sign the hard copy.
