Where do you get one and how much are they?

The fee for a single re-entry permit is 1000 baht and multiple is 3800 baht. At both airports in Bangkok there is a additional service charge of 200 baht if you do not have the form, photo and copies ready.

I assume you are going to one of the airports in Bangkok. At BKK the desk is after security just be before the immigration desks in zone 2. At DMK is just after departure immigration.
