Non O-A extension at Chaengwattana

I have to apply for a Non O-A extension at Chaengwattana in about 2 weeks and just want to check when to go about things. Few questions:

1) Can I just show up at CW as in previous years, or is an appointment required now?

2) I'm using money in bank method for the first time, deposited in Krungsri MTD account 2 months ago, do I need to take anything in addition to my bank book? Can the book be updated at CW; is some sort of bank letter needed as well? 

3) For insurance I will use LMG with 200k deductible. I just filled out the form at their head office, but they were unsure which commencement date to use. Last day of my visa is 3rd November (Tuesday) but I will probably go to CW about one week before - what date should I start the policy?
