I have a blank TM47 form and she has Thai ID.

If she attends do I sign and date beforehand or on the exact date required ?

In addition to my passport with last report are any other docs needed please ?

I give my wife my passport, a completed and signed TM47 form when she goes to do mine.

You do not have to be sick or have a particular reason for somebody to them do for you.
