TR, declaration form

I try to apply a tourist evisa online. I cannot find the declaration form http://thaievisa.go.th

May I know where can I download a blank form or it's an online form?

You download it when doing the application.

Just go the your capital cities Thai Consulate website and download it from there.....

They're basically all identical except that each Consulate has their own address at the top.

I looked on 2 embassy websites and could not find the declaration form.

Oops, my mistake..  thinking "declaration" was "application".

THere is a declaration link while doing the application (pl. see image in  @ubonjoe's post).
