Watchdog examines how Levy was managed

Two Metropolitan Police officers are facing misconduct investigations over the handling of serial sex offender Simon Levy, who murdered two women after being released while under police monitoring.

Levy, 40, was on bail when he committed a violent rape in January 2025. He went on to murder Carmenza Valencia-Trujillo, 53, in March and Sheryl Wilkins, 39, five months later.

He was convicted at the Old Bailey of two murders, two rapes and other serious offences including grievous bodily harm with intent and non-fatal suffocation. He will be sentenced on 12 August.

The Independent Office for Police Conduct (IOPC) is now investigating whether police action could have prevented Levy from reoffending and whether officers followed the required policies, procedures and training.

Serious questions over police decisions

The investigation covers the four years from Levy's first sexual assault conviction in September 2021 until his arrest in September 2025.

Levy had been released from prison on licence in February 2023 and was briefly returned to custody in May 2024 after being accused of rape and breaching his licence conditions. He was not charged.

He subsequently remained in the criminal justice system while accumulating further allegations and convictions for attacks on women.

The Met has acknowledged that failures to remand Levy in custody during court hearings in May and June 2025 left him free to commit further offences.

Two British Transport Police officers have also been moved to other duties after the force admitted "incompetence" in handling investigations into assaults involving Levy.

CPS admits it should have pushed harder

The failures extend beyond policing.

Lisa Ramsarran, chief crown prosecutor for the Crown Prosecution Service, said its actions had fallen short of the standards victims, families and the public were entitled to expect.

She said there had been points when prosecutors "could and should have taken a more robust approach" to representations concerning bail, based on the information available.

Crime and policing minister Sarah Jones said the case raised "serious questions", particularly over how Levy was able to inflict further devastating harm after being released.

The IOPC stressed that misconduct and gross misconduct notices do not necessarily mean disciplinary proceedings will ultimately follow.

But with the Met, British Transport Police and CPS all acknowledging significant shortcomings, the case is likely to raise much wider questions about how Britain's criminal justice system deals with repeat sex offenders — and whether warnings about dangerous offenders are being acted upon quickly enough.

Police are also appealing for any other women who may have been victims of Levy to come forward.
