easiest I would think is take a van to Penang and get yourself to the Penang Airport in Gelugor. from there you can the new no-frill airline from Penang to Kuala Terengganu (KT). From KT Airport you can arrange your transport to Merang jetty for the ferry to Perhentian.

If you can get a good connection it would take

Hat Yai Penang - 3hrs

Penang (airport) - 3hrs

Penang - KT via flight - 1 hr

KT - Jetty - 1 hr

Jetty - 2 hrs

if not the other option is Hat Yai take a bus to KL and take another overnight bus to Kuala Terengganu, this will be around 10 hrs + 8 hrs.

From Had yai go by train to Sungei Golok. 3-4 hours

There is supposed to be a boat from TAK BARA direct to the islands [ask at Sungei Golok]

Otherwise cross into Malaysia get a bus or share taxi about 2 hours

then get boat out to islands
