You could also try link which was given in another thread, not sure if it applies only to persons covered under the 30 baht scheme/ Thai Social Security or not: http://www.nhso.go.th/NHSOFront/SelectView...000000000000044 NHSO is the National Health Security Office it's all in Thai, pretty sure anyplace you might contact will be so need to have a Thai speaker do it if you are not fluent.
