On re entering Thailand the form asks for your visa number, Do they require your original visa number,

the current annual extension of stay or current re entry permit number or all three.

Last time in i left my re entry permit number of and had to show the officer where it was in my passport.

Is that the only number required.

When passing immigration always check if they gave you the right stamp. Mistakes do happen and it is easier to correct it on the spot.

Just to emphasize, I write it as: "RE-ENTRY PERMIT # xxxxxxx"

Include those words and more difficult, but not a all impossible, to receive a 30-day chop, so as said, BE SURE to check what you got before waling on. Not difficult to have that corrected but it does entail a trip to Immigration to do so.
