self described:

Well, it's start after I woke up in the morning for few months ago. Start from little bit til now I feel hurt a lot.. Now I can't use my right hand well, everytime I write, use keyboard and mouse I will feel pain so much in my wrist and also everytime I move my thumb I will feel hurt a lot. Especially in the morning after I woke up.. I woke up with pain on my right wrist. I did everything I can to heal myself.. see doctor, take medicine and massage.. but it didn't help me much.. I think it because of my work detail that I have to use keyboard and mouse everyday. and use my hand all day..

notes from poster:

i told her i thought it was from work

that might be why she thinks so too

makes perfect sense though

we are both very concerned and dont know what to do next

help?

thanks

It's carpal tunnel syndrome I should think. She can have a quick operation at hospital and he'll be right as rain.

:)

(note I am even more likely to be right as she is female and it is nore common in women)

Carpal tunnel syndrome can be pretty serious. Do not use your hand or wrist until you get checked out by a doctor.

See http://www.ncbi.nlm.nih.gov/pubmedhealth/PMH0001469/ for some info.

Make sure you get it checked soon as it can be damaging. A gentle not to tight wrap of an ace bandage to minimize movement may help until you get it checked out. Cost about 25 baht at the pharmacy.

Some type of repetitive stress injury sounds likely. If so, initial management is conservative: antiinflammatory drugs (NSAIDs), rest the hand to the extent possible and (very important) wear a hand splint at night. These can be bought at Boots, Watsons etc.

If these measures are insufficient, surgery may be necessary. Sometimes Steroids injection is tried before that.

She needs to see a hand specialist. That is a doctor board certified in orthopedics with specific expertise in hands and most definitley does not mean just any doctor.

If in Bangkok, I highly recommend Dr. Panupan (Prof at Siriraj and leading hand expert in Thailand/one of the foremost internationally). He has private hours at Phyathai 2 on Sunday mornings and also at Chaophya Hospital Thursdays for just an hour, I think 6 - 7 PM. In both instances call to confirm he'll be there, he travels a lot for conferences and things.

If she does not see a doctor but chooses instead to keep self-treating, she won't get into trouble from the night splint but should be sure to read up carefully on NSAID contraindications and side effects.

Massage is a bad idea and should be avoided, may aggravate matters.

Most likely DeQuervains disease or tenosinovitis of the tendons of the thumb..

Several conservative or non-invasive therapies are available but, in the worst case scenario, a small incision in the tendon sheath may be required.

Lots of info from Dr. Google...

  • 1 month later...

At the risk of rubbernecking... was I right?

The time elapsed is now such that after the doctors carried out the electric current tests, they will have concluded she has carpal tunnel syndrome, done some therapies that will have not worked, have tried out some medicines that will not have worked, and about now your friend will be up for a surgical operation. Am I right?

she says not better but not worse

i think we are both at a loss what to do

at least we have steered her away from massage

i always think of surgery as the absolute last, final, and desperate option

thanks for your concern if that is why you ask

I had a similar problem last year , woke up with the back of my hand swollen and pain in my thumb , had it x-rayed and found a small bone broken in the base of my thumb , put in a splint and was given a spray to use Yunnan Baiyou , and sublinguals of the same name ( Chinese medicine) , problem cleared up in 4 days .
