I'm not trying to be funny here, but I have heard Thais express concern simply by asking คิดมากไหม.

I guess this would be our equivalent of asking 'Do you have a lot on your mind', the assumption being that this automatically leads to headaches.

From my understanding, 'khit mak' tends to be translated into 'worry'. It's not idiomatic in any sense.
