I've just started filling in my misses FLR(M) form and i have a couple of Questions if you can help me

Q1.4 It asks for your full name in your passport

is this just the first name as it asks you for your surname or family name in Q1.5

Q6.4 About Biometrics it asking about previous time you have had them taken on the end column is asks

British diplomatic post if they was taken abroad. my misses has had them taken at vfs Bangkok do i just put Thailand ?
