Is my 90 day notification due from the date on my departure managment card (last entry date) May 26?

OR

90 days from the date of my last visa extension July 24?

From your entry. Unless extension was your first one.

Some offices do report with extension but give you a receipt giving a new report date.

If none of the above you are late making your report and subject to a 2000 baht fine.
