Last entry re-sets the 90 day clock.

If you have an entry after your last 90 day report that day is day one of a new 90 day period.

My annual extension was granted about 5 weeks after my last entry into the Kingdom.

Do I need to make a 90 day report from date of entry or from date of extension renewal? I vaguely remember reading that an extension was also considered as a 90 day report.
