Help me understand. I have an ED visa until Aug '14, issued in Pattaya.

Currently staying at Koh Samui. How to do a notification of residence here?

With a multiple-entry non-ED visa you are allowed to stay for a maximum of 90 days on each entry and therefore you need not make the 90-day notification of address. If you did not leave Thailand within the "UNTIL" date shown on your arrival stamp, you are on overstay.

An ED "visa" would be issued at a Thai consulate in another country. What did you get at Pattaya, an "Extension of Stay" for your ED visa? If so, for how long?

As Maestro pointed out, what is the "Permitted to Stay UNTIL" date in your passport?

If that's August 2014, then what you've probably missed is your 90-Day Report of residence. If you've missed these, take baht 2,000 with you when you visit Immigration. Normally you make this report at the same place where you got your Extension, but you might be able to do it at Samui if you take along a note from the hotel or guest house you are now staying at to show "residence" in Samui.

You didn't get a visa in Pattaya you got an extension of stay. Immigration does not issue visas other than those issued to get an extension.

You are apparently asking about doing a 90 day report. To do it in Samui you will need an address there to put on the TM47 and possibly proof of that address.
