A Visa allows entry and permission to stay for the validity of the Visa type. When that Visa expires, a red 'USED' stamp will be stamped across it by Immigration. Thereafter annually Immigration will stamp a 'permit' in your passport. You need a 're-entry permit' to keep your extension alive. I entered Thailand in 2011 on a Non Immigrant O (Other) Visa and I haven't left the country since. Despite an entry and a number of Extensions of stay since, there is no red Used stamp on that Visa. All Immi offices are fiefdoms and things vary. So one cannot be dogmatic about them.
